Introduction.

The purpose of this statement is to inform you what personal information S.M.A.K.E  service collects and hold, why we collect this data, how long we will keep it and any rights you have over your personal data.

Data we may hold about you could include personal information we requested from when you enquired about our services to set up an initial appointment. This may include:-

*Personal details such as your name, gender, phone number, your availability and other relevant information. 

* Health or medical Information including disabilities. 

Details are usually collected directly from you in the course of providing our services when you agreed to attend either the counselling or mediation appointment.

Our use of your Information.

Your personal information will only be used to provide you with S.M.A.K.E services and to give you information relating to our services. We will not share your personal details with any other person or organisation without your knowledge and permission.

Your rights.

You have the right to request access to information S.M.A.K.E  hold about you and to amend or ask for deletion of information that may not be accurate or correct.

Confidentiality.

A breach of confidentiality is where a person shared information with another in circumstances where it is reasonable to expect that information will be kept confidential unless we find there is  legal reason concerning the safeguarding of a child or adult  as a perceived risk of harm.  Therefore confidentiality will only be broken if:

* We believe either you or another person/child is at risk of being hurt by someone else.

* We believe that you or another person/child may be at risk.

* We believe that you maybe a risk to others or yourself.

Once a client finishes Counselling or Mediation all data collated is stored securely for 7 years and then destroyed.
